Consent to the Processing of Personal Data for Voucher Purchase and Appointment Booking

  1. By giving this consent, you authorize Faryna CZ s.r.o., registered at Sicherova 1652/7, 19800 Prague 9, Kyje, Company ID No. 02864461, registered with the Municipal Court in Prague, Section C, Insert 224511/MSPH (hereinafter referred to as the “Controller”), to process your personal data as the data controller within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as the “Regulation”). The personal data to be processed include your email address, first name, last name, and telephone number.
  2. Your email address and other personal data will be processed for the purpose of inclusion in the database for sending information regarding appointment booking, and for sending commercial communications related to the Controller’s activities. The data will be processed for the period necessary or for one year from the granting of consent, unless you extend this period or withdraw your consent earlier. Such processing is permitted under Article 6(1)(a) of the Regulation.
  3. You may withdraw your consent to the processing of personal data at any time, for example by sending an email to info@lash.cz. Withdrawal of consent will result in the cessation of commercial communications.
  4. The processing of personal data is carried out by the Controller.
  5. Please note that under the Regulation, you have the right to:
  • withdraw your consent at any time,
  • unsubscribe from commercial communications at any time,
  • request information about what personal data we process about you, request access to this data, and request that it be updated or corrected, or request restriction of processing,
  • request the erasure of your personal data, unless we are required or authorized to continue processing such data under applicable legal regulations,
  • the right to data portability and to request a copy of the processed personal data,
  • effective judicial protection if you believe that your rights under the Regulation have been infringed as a result of the processing of your personal data in breach of the Regulation,
  • in case of doubts about compliance with obligations related to the processing of personal data, to contact the company or the Office for Personal Data Protection.
